Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that supports research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 which allows for multiple and varied meta-epidemiological research studies to evaluate the effect of treatment on trials that employ different levels of pragmatism as well as other design features.
Background
Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision-making. However, the use of the term "pragmatic" is inconsistent and its definition and evaluation requires further clarification. Methods

Koppenaal and colleagues found that 36% of the 89 pragmatic studies were placebo-controlled, or conducted prior to the licensing. They also found that the majority were single-center. Therefore, they aren't quite as typical and can only be described as pragmatic when their sponsors are accepting of the lack of blinding in such trials.
Additionally, a typical feature of pragmatic trials is that the researchers attempt to make their findings more valuable by studying subgroups of the sample. This can lead to unbalanced comparisons with a lower statistical power, thereby increasing the chance of not or misinterpreting the results of the primary outcome. In the case of the pragmatic trials that were included in this meta-analysis this was a significant problem because the secondary outcomes were not adjusted to account for the differences in baseline covariates.
Furthermore the pragmatic trials may have challenges with respect to the collection and interpretation of safety data. This is due to the fact that adverse events are generally reported by the participants themselves and are prone to reporting delays, inaccuracies or coding deviations. It is therefore important to enhance the quality of outcomes ascertainment in these trials, ideally by using national registries rather than relying on participants to report adverse events in a trial's own database.
Results
While the definition of pragmatism does not mean that trials must be 100 100% pragmatic, there are advantages to including pragmatic components in clinical trials. These include:

However, pragmatic trials can also have drawbacks. The right kind of heterogeneity, for example could allow a study to generalise its findings to many different settings or patients. Several studies have attempted to classify pragmatic trials using different definitions and scoring methods. Schwartz and Lellouch1 developed an approach to distinguish between explanatory trials that confirm a clinical or physiological hypothesis as well as pragmatic trials that aid in the selection of appropriate therapies in the real-world clinical setting. The framework was comprised of nine domains that were evaluated on a scale of 1-5, with 1 being more lucid while 5 was more practical. This distinction in the main analysis domain could be explained by the fact that the majority of pragmatic trials analyse their data in the intention to treat way however some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the areas of organization, flexible delivery, and follow-up were merged.
