Pragmatic Free Trial Meta

Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It shares clean trial data and ratings using PRECIS-2, permitting multiple and varied meta-epidemiological research studies to evaluate the effect of treatment on trials that employ different levels of pragmatism and other design features.

Background

Pragmatic trials are increasingly acknowledged as providing evidence from the real world for clinical decision-making. The term "pragmatic" however, is a word that is often used in contradiction and its definition and measurement require clarification. Pragmatic trials are designed to inform clinical practices and policy choices, rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should aim to be as similar to real-world clinical practice as possible, such as the recruitment of participants, setting and design, the delivery and implementation of the intervention, and the determination and analysis of outcomes and primary analysis. This is a major difference between explanatory trials, as described by Schwartz & Lellouch1 which are designed to prove the hypothesis in a more thorough way.

Trials that are truly pragmatic must be careful not to blind patients or the clinicians as this could lead to bias in the estimation of the effect of treatment. Practical trials should also aim to attract patients from a wide range of health care settings, to ensure that the results can be compared to the real world.

The difference in the analysis domain that is primary could be explained by the fact that the majority of pragmatic trials process their data in an intention to treat way while some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the domains of organization, flexible delivery, and following-up were combined.

It is important to remember that a pragmatic study should not necessarily mean a low-quality study. In fact, there are a growing number of clinical trials that use the term 'pragmatic' either in their abstracts or titles (as defined by MEDLINE but which is not precise nor sensitive).
